Sharing Soulprints

Hospital to Home Visitation Program

"A soulprint is an imprint on a person's heart that flows through to their soul and leaves them forever changed."

There are times when a friendly face or helpful hand makes all the difference. Sharing Soulprints provides volunteer assistance to those who are chronically or acutely ill, and older adults who are alone and in need. Trained volunteers provide services coordinated by a hospital case manger and may include:

  • Friendly visits
  • Errands and shopping
  • Transportation for medical appointments in partnership with The Caregivers Inc.
  • Telephone reassurance
  • Active listening and companionship

Volunteers visit with patients and provide social support while they are in the hospital and after they return home. A regular friendly visitor can help monitor those at risk and help to prevent unplanned hospital readmissions. Outside referrals to the program may be made through the Roger Dionne, MD, Senior Center or Milette Manor Parish Nursing Center.

Sharing Soulprints is a cooperative program of the Roger Dionne, MD, Senior Center, the Health Ministry/Parish Nurse Program and the Volunteer Department at St. Joseph Hospital.
