The case management team at St. Joseph Hospital will visit you and your family early in the appointment process or hospitalization. They develop a care plan that includes you as the primary decision maker, providing you with a wide variety of options. Our team will assist your needs during an appointment, hospitalization, and even after you are discharged – ensuring that you receive the proper continuation of care. The main function of our case management team is to act as advocates for you to your healthcare team, promising that your individual healthcare needs and goals are met.

Discharge Information

What differentiates the case management team at St. Joseph Hospital from others, is the assistance that we provide after discharge. We’re available to help you – even after you leave an appointment or hospitalization. As a part of our services, a member of our team will call you after discharge to confirm that your medical needs were met. It is important to contact a member of our team if any of the following situations occur:

  • The home healthcare agency did not schedule a follow-up with you
  • The healthcare equipment that you were expecting did not arrive as scheduled
  • You discover that you need more help than you anticipated
  • Acute rehabilitation

  • Information on Federal Medicare coverage and medical benefits – Patients with commercial insurance plans including Medicare replacement may be directed to their individual carriers, as each varies in coverage
