Discharge Information

We’re available to help you – even after you leave an appointment or hospitalization. As a part of our services, a member of our team will call you after discharge to confirm that your medical needs were met. It is important to contact a member of our team if any of the following situations occur:

• The home healthcare agency did not schedule a follow-up with you
• The healthcare equipment that you were expecting did not arrive as scheduled
• You discover that you need more help than you anticipated
