Case Management

At St. Joseph Hospital, our case management team offers services to every outpatient and inpatient across all of our locations. Our case managers work closely with your physicians, nurses, and other necessary members of your healthcare team to ensure that you are efficiently receiving the services that you need to be the healthiest version of yourself.

How We Help

The case management team at St. Joseph Hospital will visit you and your family early in the appointment process or hospitalization. They develop a care plan that includes you as the primary decision maker, providing you with a wide variety of options. Our team will assist your needs during an appointment, hospitalization, and even after you are discharged – ensuring that you receive the proper continuation of care. The main function of our case management team is to act as advocates for you to your healthcare team, promising that your individual healthcare needs and goals are met.
