Congratulations to our Case Management Team who recently won the 2022 Honoring Excellence in Patient Safety & Quality Improvement Award from the New Hampshire Hospital Association!
This award is given to a hospital that has performed at a high level of accountability and has been instrumental in the following areas:
- Improved outcomes due to a strong culture of safety
- Reduced patient harm events
- Reduced health disparities among specific patient populations
- Supported a learning culture committed to trust, feedback, and higher problem solving
