NCQA PCMH Recognition

St. Joseph Hospital and its five convenient locations in Nashua, South Nashua, Hudson, Merrimack, and Milford, have been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.

To receive this recognition, practices must meet certain criteria of a Patient-Centered Medical Home ensuring patients have:

  • Access to care
  • Care coordination
  • Case management
  • Tracking of test results/referrals
  • Continuous quality improvement
  • Patient experience surveys/improvements

What is NCQA?

NCQA is a private, non-profit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. NCQA‘s Healthcare Effectiveness Data and Information Set (HEDIS) is the most widely used performance measurement tool in health care. NCQA‘s website contains information to help consumers, employers, and others make more informed health care choices.
