What is an Ambulatory Care Pharmacist?

An ambulatory care pharmacist is an expert in medication management. They work closely with patients and their healthcare providers to address medication needs through a collaborative, patient-centered approach. Additionally, ambulatory care pharmacists optimize medication regimens through initiation, adjustment, and/or de-prescribing of medications.

Role of Ambulatory Care Pharmacist

Ambulatory care pharmacists play an integral role in our patients needs, as they work with complex health conditions such as diabetes, high blood pressure, high cholesterol, and asthma/COPD.

Beyond medication management, ambulatory care pharmacists provide medication education, lifestyle coaching, and patient advocacy. They may even order laboratory tests and perform limited physical assessments, including blood pressure measurements or comprehensive foot examinations for patients with diabetes.

Ambulatory Care Pharmacist Program

Leading the program is Andrew Barton, PharmD, BCPS, BC-ADM, CDCES, specializing in diabetes education and management. He reviews medical records, assesses medication safety, and offers recommendations in collaboration with patients primary care providers.

Andrew’s expertise as a Board Certified Pharmacotherapy Specialist (BCPS) and Certified Diabetes Care and Education Specialist (CDCES) includes a background in medical writing regarding diabetes pharmacologic therapy, making him uniquely qualified to provide the best education possible to our patients.

Scheduling Information

The Ambulatory Care Pharmacist Program is designed to provide patients with flexible scheduling options, allowing them to schedule their visits with Andrew while at their appointment with their primary care provider, or call on their own. It is also designed to help patients of all ages and backgrounds, as we strive to provide personalized care and support to our patients.
