St. Joseph Hospital is continuously advancing to fulfill the healthcare needs of the community. As part of this effort, we are pleased to announce the launch of the Ambulatory Pharmacist Program at St. Joseph Hospital Family Medicine & Specialty Services in Milford, New Hampshire.

Patients can take advantage of this program, where comprehensive diabetes education will be provided, including management therapy strategies and lifestyle modifications.

About the Program

Leading this program is Andrew Barton, PharmD, BCPS, BC-ADM, CDCES, a highly skilled and experienced pharmacist who specializes in diabetes education and management. Andrew will be reviewing each patient’s medical records, assessing medication safety, and offering recommendations for optimized therapy in collaboration with their primary care providers.

Andrew’s expertise as a Board Certified Pharmacotherapy Specialist (BCPS) and Certified Diabetes Care & Education Specialist (CDCES) includes a background in medical writing regarding diabetes pharmacologic therapy, making him uniquely qualified to provide the best care possible.

Scheduling Information

The Ambulatory Pharmacist Program is designed to provide patients with flexible scheduling options, allowing them to schedule their visits with Andrew while at their appointment with their primary care provider, or call on their own. It is also designed to help patients of all ages and backgrounds, as we strive to provide personalized care and support our patients.
