Ambulatory Pharmacist Program

St. Joseph Hospital is now offering an Ambulatory Pharmacist Program at Milford Medical Center. Available Monday through Friday, this program will provide patients with comprehensive diabetes education, including management therapy strategies and lifestyle modifications.

This program is run by Andrew Barton, PharmD, BCPS, CDCES, who will be providing his vast expertise in diabetes education and management to patients. Andrew will be reviewing patient profiles, assessing medication safety, and making recommendations for optimized therapy in consultation with the patients’ primary care providers. Andrew’s expertise as a Board Certified Pharmacotherapy Specialist (BCPS) and Certified Diabetes Care & Education Specialist (CDCES) includes a background in medical writing regarding diabetes pharmacologic therapy, making him uniquely qualified to provide the best care possible.

This program is designed to provide patients with flexible scheduling options, allowing them to schedule their visits with Andrew while at their appointment with their primary care provider, or call on their on. It is also designed to help patients of all ages and backgrounds, as we strive to provide personalized care and support to our patients.
